Direct Return Authorization (DRA) Warranty Repair Form

Note: This Warranty Repair Form is intended for resellers only and applies only to the Americas geography - the United States, Mexico, Canada, and Latin America. Please read all instructions carefully.

Direct Return Authorization (DRA) Warranty Repair Procedure
For Intel® Motherboards, Systems, and Servers

To Request a Warranty Repair:

  1. Complete all required fields of the DRA Warranty Repair Form (see above) and click on submit button.
    1. Please provide the following information when completing the form:
      1. Company name, contact person, email address, telephone number, and complete shipping address.
      2. If available, include technical contact, email address, and telephone number (technician or engineer using the product).
      3. Serial number, part number, and problem description.
    2. Please indicate whether or not the serial number must be maintained (warranty products only).
      1. If "No" is selected, the original product received will be replaced if product is determined unrepairable. This depends upon product availability. Note: Refurbished product is used for replacements.
      2. If "Yes" is selected and the board is unrepairable, the original product will be returned to the customer unrepaired.
  2. Upon receipt of the completed DRA Repair Form, a Direct Return Authorization Number (DRA number) will be assigned within 24 business hours after submitting the request and returned to you via email.
  3. Shipping charges including GST, PST, VAT, customs clearance, or other expenses to and from the U.S. repair centers are the customer's responsibility. Products still under warranty will be returned to the customer with freight prepaid by Intel's preferred carrier.
  4. Once the product has been authorized by US Post Sales Support (USPSS), clearly mark the outside of all shipping containers with the DRA number.
